Owasippe Leaders’ Packet If your troop was unable to attend a Leaders’ Meeting, or if you require extra copies of any paperwork, all of the forms that are included in the Leaders’ Packet can be found below. Manuals The Owasippe Leaders’ Manuals, completely rewritten for 2012, contain all of the information needed for a successful stay at camp. Everything from where to park to comprehensive programming descriptions can be found in this document, specific to the section camp where your troop will be staying. Have a question about summer? The answer is likely in the manual. Payment Forms This form is the first step in the reservation process. This form, complete with appropriate payment, is due February 28th, 2012 or at a Leaders’ Meeting. This form, complete with appropriate payment, must be postmarked by May 31st, 2012 in order to avoid a late fee. This form should be submitted by April 15, 2013. Decisions will be made by May 15, 2013. Only NEW SCOUT applications may be submitted after June 30th. When available funds are fully allocated, a waiting list will be maintained. Health Forms Any person staying in camp for more than three nights must submit a medical form, and complete the medical re-check process. This form details any medications that Scouts in your troop will be consuming during their stay at camp. Please complete prior to your arrival at Owasippe. Youth Protection Forms Please read this important information with regards to the now-mandatory background check for any individual in camp over the age of 21. This is the background check form for Illinois residents. This is the background check form for Indiana residents. This is the background check form for Michigan residents. If a Scout will be leaving camp during the week for any reason, an Early Release form must be signed by a parent or guardian prior to the Scout leaving the property. All leaders must fill out and sign this form and bring it with them to camp. Program Forms If anyone in your troop might participate in horse programming, this form must be printed, completed, and signed by a parent or guardian prior to attending any program whatsoever at the Diamond O Ranch. If you are unsure if your Scouts will want to participate in the horse program, please have their parents complete the waiver just in case. Be prepared! Complete this form if any camp attendees will be inducted into the Order of the Arrow at camp. Use this form to help organize the swimming classifications for Scouts in your troop. Planning This form includes equipment and dietary needs for your troop, and must be completed and sent to camp two weeks prior to your arrival at camp. All troops must complete this form and submit it upon arrival at camp. This is a great resource for Scouts when they are planning what merit badge classes they will take at camp.
